Community Wellness Outreach Wokingham

The Community Wellness Outreach project seeks to support residents of Wokingham that may be of higher risk and don’t necessarily attend the General Practice. The health checks are a full NHS check, and we also work with “Involve” to provide a social prescribing / community navigation service - providing more in-depth support to those that have issues that lead to high blood pressure like mental wellbeing / finances/ carer responsibilities.
